Job Opening

Position:     Registered Nurse  (R.N.) – Full Time Location:  Greater South Shore area.   Summary:    We are looking for a full time Registered Nurse to support our Adult Foster/Family Care Program in the Greater South Shore area. This position is exempt.   Position Overview:
  • Reporting to the Family Supports Program Director, the AFC Nurse responsibilities include, but are not limited to, the following: Complete nursing assessments and coordinate all other applicable clinical assessments.
  • Develop and review each member’s AFC care plan annually or upon significant change and review with member’s physician.
  • Select, train, evaluate, and supervise AFC caregivers in conjunction with the Care Manager.
  • Monitor and document the health status of all members.
  • Report changes in health status to member’s physician.
  • Conduct on-site visits with each member monthly, bimonthly or more often as members condition warrants.
  • Review caregiver logs
  • Submit semi-annual health status report to the members PCP.
  • Plan for and implement discharges from the AFC Program
  • Conduct initial orientation and ongoing trainings for AFC caregivers.
  • Educate the member about hygiene and health concerns
  • Coordinate the implementation of physician’s orders with member, AFC caregiver, and AFC multi-disciplinary team.
  • Report all cases of neglect or abuse to the appropriate agency.Requirements and Responsibilities
  • Current RN Licensure in Massachusetts
  • A Bachelor of Science degree in nursing and at least 1 year clinical experience and or nursing in the community or long term setting, or Diploma RN with 2years of clinical experience in  community or long term setting.
  • At least two years of experience in the direct care of the elderly or disabled adults.
  • A valid driver’s license, able to provide own transportation, and spend a significant amount of time in the car.
  • Excellent people, communication, organization and writing skills.
  • Must be able to work independently with a flexible schedule, which may include nights and weekends.
  • Serve on a rotating on call system for families.
